This template is extremely easy to use. If you need to add more lines, just copy a row and insert the copied row under the appropriate section.
I created this due to a request by a Vertex42 fan. Note that I am not a medical doctor, and I am lucky enough to not need to use this template myself (yet). So, if you are using this template and would like to see something changed or added, please let me know. Comment via the related blog article.
